4C Health offering Primary Care in Logansport, Rochester, and Peru

4C Health has added Primary Care to its service offerings as part of the mission to provide high quality, whole healthcare to rural community residents of Cass, Miami, and Fulton counties. Blending behavioral health services with primary care creates access, quality, and convenience for residents versus traditional models that require patients to travel to multiple location for multiple different healthcare services.

Primary Care services began in Logansport, Indiana in December 2023 and in Rochester, Indiana in August 2024. These services are now expanded to Peru, Indiana effective October 2024.
